Root Cause Analysis
Root Cause Analysis is a method that is used to address a problem or non-conformance, in order to get to the “root cause” of the problem. It is used so we can correct or eliminate the cause, and prevent the problem from recurring.
Root Cause Analysis (RCA) is a management process that seeks to locate the ultimate cause or 80/20 rule causes behind performance or process-related problems in a business or engineering environment, and then proceed to resolve the problem by treating these underlying causes.
Within an organization, problem solving, incident investigation and root cause analysis are all fundamentally connected by three basic questions: What's the problem? Why did it happen? and What will be done to prevent it?
